After you've downloaded the Membership Application (65k PDF), please follow the instructions below:

1. Please print the application.

2. Neatly complete with a black or blue pen: Name, both mailing and physical street address; phone number, social security number and date of birth.

3. Sign the application where your "Signature" is required.
4 . Copy unexpired, valid govenment issued identification card bearing a photograph (i.e. U.S. driver's license, passport or alien registration card); and copy of two of the following forms of identification:

Social Security Card, School Identification Card, Major Credit Card, Current utility bill showing current address or Current pay stub showing current address.

5. Mail the application and copies of ID to:
