After you've downloaded the Membership Application (65k PDF), please follow the instructions below:

1. Please print the application.

2. Neatly complete with a black or blue pen: Name, both mailing and physical street address; phone number, social security number and date of birth.

3. Sign the application where your "Signature" is required.

4 . Copy an unexpired, valid govenment issued identification card bearing a photograph (i.e. U.S. driver's license, passport or alien registration card); and copy of two of the following forms of identification:

Social Security Card, School Identification Card, Major Credit Card, Current utility bill showing current address or Current pay stub showing current address.

5. Mail the application, along with $5.00 (the amount required to remain on deposit in a savings account to qualify for WRCU membership) and copies of ID to:
